In the event of lost or damage to articles described on this form, I agree to pay the regular price of replacement value on all items listed. I agree to pay reasonable collection fees plus attorney’s fees in the event a suit is instituted for recovery of, or damage to rented costumes and accessories. I am aware that the rental is for the agreed date only and must be returned on agreed date listed below. I understand and agree to pay $15.00 per day late charge per costume for each day the costume and accessories are kept past the agreed date. I am aware there are no cancellations, which means no refunds. I understand that the assurance fee guarantees careful and proper care of rentals while it assigned to me. This fee in no way covers the total replacement cost of said rentals. I am aware of the risk and hazards related to this transaction and hereby elect to assume voluntary and full responsibility.
